Multiple Same Choice (MSC) voter instructions would be something as the
following--

Put a 0 (zero) next to the name of the candidate or candidates who you do not
want to be elected.
Example-
John Doe    0

Put a 1 (one) next to the name of the candidate who is your first choice or
the names of the candidates who are your first choices, if any.
Example
Mary Smith   1
Tom Brown    1

Put a 2 (two) next to the name of the candidate who is your second choice or
the names of the candidates who are your second choices, if any.
Example
Sam Harris    2
Fred Allen      2

In like manner, put a 3 (three), a 4 (four), and so forth next to the name of
the candidate who is or the candidates who are your third, fourth, and so
forth choice(s), if any.
Example
Bob Morgan  3
Al   Thomas  4
Harry Carnes  4
Jim Thompson 5

You may put a 0, a 1, a 2, a 3, a 4 and so forth after the name of more than
one candidate.

Do not put more than one number after the name of any candidate. 

[If NOTA is being used] If you not want any of the candidates to be elected,
then put a 0 (zero) after None Of The Above (NOTA).
