[EM] New version of n-rank ballot

Hi all,

Here's a new version of the ranked ballot:

Changes:
*  Uses select drop down form elements instead of plain text for 
ratings, allowing for minimal operation without Javascript.
*  Allows for altering the number of stars (between 1 and 100)
*  Streamlined sorting.  All columns now have a single sort order.  This 
really mattered for "Rating", which now defaults to a descending order 
sort.  Additionally, in case of tie, the previous sort order is retained.
